For the Hospital's Name:

- USE CAPITAL LETTERS

> CALAMBA MEDICAL CENTER, CALAMBA CITY, LAGUNA

> MARY MEDIATRIX MEDICAL CENTER, LIPA CITY, BATANGAS

> MANILA ADVENTIST MEDICAL CENTER, PASAY CITY

For the Student's Name:

- USE CAPITAL LETTERS

> FIRST NAME, MIDDLE NAME, LAST NAME Format

SIGNATURE ABOVE NAME

Patient's Initials:

- Separated by Period

> Ex. J. A .R

- No need to write the Number Sign (#)

- Write the CASE NUMBER or OUT PATIENT or MEDICAL MISSION

> Ex. 14344

Surgical Procedure Performed:

- Use Operation, Site, Right/Left Format

- First LETTER should be CAPITAL (Correct Spelling Please!)

> Ex. Excision Breast Mass, Right

OR Nurse:

- Write their names WITH MIDDLE INITIALS

- NO NEED to write RN

- NO NEED to write their license numbers

Clinical Instructor:

- Write Mercy S. Tejada, RN, MSN, only
