SUPER DOOPER EXTRA NOTES:
1. YOU are NOT REQUIRED to buy erasable PENS!! I just stated that as ONE OF THE OPTIONS. USE PENCIL or anything else, or if your sure enough with your data then do write it with a PEN.
2. the BLUE COLORED words are COLORED just for EMPHASIS. and it doesn't mean that the supposed data to be written on those spaces are also COLORED BLUE. EVERYTHING SHOULD BE IN BLACK COLOR.
